Schedule:
Full-time Monday - Friday Occasional weekends and evenings required.
Responsibilities:
Participate in the admissions/discharge process by ensuring all required documentation and assessments are completed.
Advocate for residents by assisting them with assertion of rights within the facility.
Assist Residents and families with financial and legal matters, applying for services, Medicaid application process, etc.
Meet the needs of Residents who are coping with stressful events.
Act as a liaison with local hospitals and care providers to ensure successful admission to the facility.
Understand MDS Process with ability to interview, input, and complete social service assessments.
Provide tours, distribute facility brochures, and offer support and information to families as needed regarding the admissions process.
Work closely with other departments to assure smooth transition of Residents from Skilled Nursing to other levels of care such as Assisted Living, Home, or Long Term Care.
Other duties as assigned.
Qualifications:
At least 1 year of job-related experience in Long Term Care/Skilled Nursing setting preferred but not required.
